[EP 223] Radical Antegrade Modular Pancreatosplenectomy for Pancreatic Neck-Body Insulinoma

Background : Insulinoma is a rare pancreatic islet cell tumor, the most common cause of hypoglycemia related to endogenous hyperinsulinism, occur in 1-4 people per million. Insulinoma account for 60% of islet cell tumors (ICT) and are typically hyper vascular, solitary small tumors, 90% of which measure less than 2cm and 30% measuring less than 1 cm in diameter.
Methods : We report 52 years old man with the lightheadedness, shakiness and loss of consciousness. In November of 2022 patient did not wake up in the morning. Family members called the emergency and they told he lost consciousness due to low glucose levels. In October of 2023 he has got recurrent attacks of coma. District hospital recommended him to do CECT and small lesion was diagnosed at the neck and body of pancreas and he presented to our hospital. Serum insulin – 106 mIU/L C-peptide 7.8 ng/ml
Results : We have planned to do by laparoscopic approach but It was not possible due to the specialty of profession of the patient. There was a lot of adhesion due to the fact that the patient practiced a lot of martial arts. In the evening of the day of surgery, the patient's glucose level increased and was discharged on the 8th postoperative day without complications.
Conclusions : Insulinoma usually presents with dizziness and fainting due to repeated episodes of hypoglycemia. The diagnosis of this disease can be confirmed on the basis of repeated tests. As the disease is mainly confused with fainting convulsions, it is sometimes confused with neurological diseases.
