[EP 207] Disease Characteristics of Chronic Pancreatitis: Experiences from Hepatopancreatobiliary Centre in Sri Lanka

Background : Chronic pancreatitis (CP) is a long-term, often debilitating condition.

Methods : A prospective cross-sectional study among 110 patients with CP presented to a single Hepatobiliary-pancreatic surgical unit.

Results : The mean age was 40(range=9-74 years). The majority were males (n=84, 76.4%). The mean duration of diagnostic delay is 3.5(0-29 years) from symptom onset. Pain was the commonest symptom (n=106,96.4%) with the mean pain score was 8.5 (range 2-10). Other common symptoms were back pain, weight loss,diarrhoea which were seen in 79 (73.8%), 81 (77%), 29 (27.6%) respectively. Half (n=53) had endocrine failure. The majority of CP 60 (54.5%), was idiopathic in origin, whereas alcohol, smoking were seen in 42 (38.2%), 28(25.5%) respectively. Imaging showed main pancreatic duct(MPD) dilatation in 70 (78.7%) patients with a mean diameter of 7.9mm(range=1-32mm). Parenchymal calcifications,MPD calculus, pseudocysts were seen in (n= 57, 63.3%), 36 (42.9%),29(26.1%) respectively. There were 16(18.2%) patients with pancreatic mass lesions, and half of them were malignant(n=8,9.1%). The majority (n=85, 83%) had pain relief after initial treatment, but relapse was seen in 61 (59%) of them. Fifty (55.6%) patients had ERCP+MPD stenting, 16(18%) had ESWL, and 21 (22.3%) were on neurotropic drugs. Pancreatic enzyme supplements was required in 40(41.7%).

Conclusions : An idiopathic origin for CP was seen among a majority in this geographical area. A high degree of suspicion and early referral to a designated centre are warranted since the diagnosis is commonly late. Optimizing risk factors, compliance with the treatment, and having close follow-up are necessary for CP patients.
