[EP 163] Clinical Outcomes of Robotic PD after NAC for Patients with PDAC

Background : Patients whose primary tumor involves the surrounding vasculature were considered poor candidates of surgical resection. With an emerging consensus of extending the resectability criteria with the implementation of neoadjuvant chemotherapy, the adequacy and safety of performing minimally invasive surgery on such patients remains to be investigated.

Methods : We retrospectively reviewed 224 patients who underwent robotic (N=15) or open (N=209) surgery following neoadjuvant chemotherapy at SNUH from 2015 to 2023. Propensity score-match analysis based on TNM staging was performed to adjust for clinical differences.

Results : There were no significant differences in age and gender between the two groups, with the mean age being 62.2 years and 62.7 years, and 53.6% male and 46.7% male in open and robotic surgical patients respectively. FOLFIRINOX was the main neoadjuvant chemotherapy regimen (88.5% open, 93.3% robotic). Hospital stay was significantly reduced with robotic surgical patients, with a median stay of 9.8 days compared to 12.8 days (p= 0.01) in open surgical patients. Post-operative complication rates were similar between the two groups- 61%, and 60%, with CR-POPF detected in 9 patients (4.3%) and none in open and robotic surgery respectively. The 2-year survival rates were 73% and 84.6% (p= 0.915) before PSM analysis, and 70.6% and 84.6% (p= 0.84) after PSM analysis for open and robotic surgery respectively.

Conclusions : With shortened hospital stay, and no differences in complication, mortality and survival, this study proved the feasibility of performing MIPS on BRPC in trained centers.
