Background : There is some data regarding the increased risk of cancer, including PDAC, in patients with previous cholecystectomy. There is scarce data about the influence of oncologic outcome in this group of patients, so it’s uncertain whether prior cholecystectomy is associated or not to oncological outcomes in PDAC.

Methods : This is a retrospective analysis including 1290 patients with pathology confirmed PDAC in Severance Hospital (period from 1992-2021) and looked if previous cholecystectomized (PC) patients had a different oncological outcome than non-previous cholecystectomized (NPC) patients based on OS, RFS as main outcomes, and pathologic factors such as pT, pN, PNI/LVI and margin status as secondary outcomes.

Results : A total of 32 patients (2.7%) had previous cholecystectomies. There we no statistical difference in terms with preoperative Ca 19-9 levels (288 U/ml vs 156 U/ml, p=0.48), pT stage (p=0.81), pN stage (p=0.38), margin status (p=0.14) or PNI(p=0.77 ), and LVI (p=0.14) between NPC and PC groups. There wasn’t significant difference in OS or RFS between NPC and PC groups (mOS: 64 m vs 41 m, p=0.5; mRFS: 46 m vs 30 m, p= 0.38).

Conclusions : Previous cholecystectomy did not influence on short-term and long-term oncologic outcome in patients with resected pancreatic cancer. Further study based on larger study population is mandatory.
