[EP 128] What Are the Actual Prognostic Factors for AOV Carcinoma? Find the Pathological Risk Factors Associated Histologic Precursor Lesion.

Background : Ampulla of vater (AOV) carcinoma has high resectability rate and good prognosis in periampullary cancer. However, tumor recurrences frequently remain the primary problem after surgical treatment. In several previous studies, no factors other than advanced T stage and lymph node involvement have been clearly identified as associated with poor prognosis in AOV carcinoma. The purpose of this study is to find pathological risk factors in addition to TNM stage.
Methods : The clinical data of patients with ampullary carcinoma were analyzed. Our pathological data of AOV carcinoma reported tumor size and T and N stage and tumor associated precursor lesion and Tumor infiltrating lymphocyte(TIL) and histologic type. The association of other risk factors with precursor lesion and TIL and histologic type were analyzed.
Results : A total of 56 patients who underwent curative intent surgery between 2018 and 2022 were included in the study. The precursor lesion of total patients included tubular adenoma, PANIN(Pancreatic intraepithelial neoplasia), BILIN(Biliary intraepithelial neoplasia). Pathological risk factors were not significantly associated with recurrence free survival. However, high grade precursor lesion and histologic subtype and TIL were significantly correlated with lymph node invasion and lymphovascular invasion.
Conclusions : Our analyses of the correlation between pathological risk factors of AOV carcinoma showed that these factors were prognostic indicators. It is necessary to analyze the pathological types of large number of patients.
