[EP 055] Safe Right Lobe Liver Resection

Background : While doing liver resection, we need to take care of functional residual liver volume. This is even more important in cirrhotic liver. In case of right lobe liver resection we remove bulk of the liver and only left lobe is left. If whole of the right lobe of the liver is occupied by tumor, cyst or non functional liver area then left lobe of the liver is already either enlarged or it has taken the burden as compensation. Our case is a classic example where tumor was in the right lobe of liver and almost no functional liver tissue was left. Left lob was very much developed.Right lobe of liver can be easily removed if it is all is replaced by tumor or other pathology subjected to the condition and volume of left lob of liver.

Methods : A 70 years old male was admitted for Liver Tumor surgery. He was a known case of hepatitis b virus. His liver status was Child A. There was no co morbid factors. Radiological investigations confirmed a large tumor occupying whole of right lobe of liver. Left lobe was almost normal. Operation was done and tumor removed successfully. There was uneventful recovery.

Results : Patient was discharge on 5th postoperative day.

Conclusions : While planning liver resection, remaining functional liver volume is always of great concern. But if tumor is occupying whole of the right lobe of the liver and left lob is health then it is safe to remove the right lobe of liver. Main reason is that left lobe has already taken the control of liver function. So it is safe to remove the Rt lobe of liver without any calculation of residual liver volume.
