[EP 042] Comparison of Endoscopic Ultrasound-Guided Tissue Acquisition for Solid Liver Masses: Menghini Needle with a Lateral Forward Bevel vs. Franseen Needle.

Background : EUS-FNAB (Endoscopic Ultrasound-Fine Needle Aspiration Biopsy) of liver tissue, has recently garnered increased attention. However, there remains controversy over which type of needle has superior diagnostic performance. This study aimed to evaluate the performance and safety of two commonly used EUS-FNAB needles for the diagnosis of pancreatic lesions when applied to liver lesions.
Methods : We retrospectively analyzed patients who underwent EUS-FNAB for solid liver lesions between November 2014 and September 2023. The study evaluated two different needles: a Menghini needle, which has a core trap with a lateral forward bevel, and a Franseen needle.
Results : 66 patients were analyzed: the overall technical success rate was 100.0%, the overall FNAB diagnosis rate was 83.3%, and the overall adverse event rate was 3.0%. Tissue specimen retrieval rates for the Menghini and Franseen needle groups were 67.5% and 96.2%, respectively (p=0.006), while adverse event rates were 2.5% and 3.8%, respectively (p=0.674). The FNAB accuracy of the Menghini needle group was 89.7%, with a sensitivity of 89.5% and specificity of 100.0%. In contrast, the Franseen needle group exhibited an FNAB accuracy of 92.6%, a sensitivity of 92.3%, and specificity of 100.0%.
Conclusions : The Franseen needle demonstrated better tissue sample retrieval than the Menghini needle for solid liver lesions. No other significant performance differences were observed between the two needles.
