Background : Selecting the appropriate preservative solution is crucial for achieving success in liver transplantation. The efficacy of preservative solutions is still under debate. This study aimed to demonstrate the survival outcome between HTK and UW solutions.

Methods : Clinical and pathological data were retrospectively collected for 177 patients who underwent deceased-donor liver transplantation between January 2015 and May 2023 in Srinagarind Hospital. The preservative solution was randomly chosen according to its availability. The HTK group consisted of 110 patients, while the UW group included 67 patients. Graft and overall survival were analyzed to determine their effect using the Kaplan-Meier method with a log-rank test and the Cox proportional hazard model. Propensity score matching (PSM) and inverse probability treatment weighting (IPTW) techniques were utilized to balance confounding factors.

Results : After PSM-IPTW adjustment, there was no difference in graft survival or overall survival between the two groups. The 1- and 5-year graft survival rates in the HTK group were 91.5 and 86.9%, respectively, compared to the UW group, which were 93.7% and 91.2% (P = 0.812). In terms of the overall survival rate, the 1, and 5-year HTK groups were 89.8% and 76.9%, respectively, while the 1, and 5-year UW groups were 84.9% and 78.7% (P = 0.800).

Conclusions : The HTK solution demonstrated comparable graft survival and overall survival rates to the UW solution in the context of liver transplantation. The HTK solution could potentially serve as a cost-saving alternative.
