[BP VE 27] RBOTIC ENUCEATION OF SOLID PSEUDOPAPILLARY NEOPLASM BODY OF PANCREAS

Background : Solid pseudopapillary pancreatic neoplasia is usually a large well circumscribed pancreatic mass, with cystic and solid areas more frequently found in young women. It is a benign pancreatic neoplasia in most cases, therefore minimally invasive surgery could be a better approach.
Methods : We present ,a case of 21 yrs female ,presented with upper abdomen pain. CECT abdomen showed 39x36mm well defined heterogenous cystic mass in body of pancreas. Distance from lesion and MPD was 3mm.She was planned for Robotic enucleation of lesion. Diagnostic laparoscopy was done showing exophytic mass of size 4x3 cm arising from anterior superior surface of pancreas. Da Vinci SI system was docked using 3 Robotic arms. Lesser omentum opened pancreas exposed, inferior pancreatic attachments released and lesion was dissected all around. Lesion was carefully enucleated .Pancreatic ductal continuity was confirmed with IOUS. Hemostasis confirmed and drains placed
Results : Patient was started on diet after 6hrs of surgery.DT was removed on 3rd post operative day and discharged.Biopsy showed features of solid pseudopapillary neoplasm with negative margins.A follow up CECT done after 1 year showed no evidence of recurrence
Conclusions : Robotic enucleation of SPEN of pancreas is feasible and safe option for benign neoplasm, thereby preserving exocrine and endocrine functions of pancreas without increased risk of recurrence.
