[BP PP 9-S1] Clinical Significance And Accuracy of Continuous Glucose Monitoring in Pancreatectomy Patients

Background : After pancreatectomy, blood glucose levels fluctuate between hypoglycemia and hyperglycemia, requiring frequent monitoring and adjustment. This study aimed to determine the accuracy and acceptability of a continuous glucose monitoring system compared to the intermittent capillary glucose test in patients who have undergone pancreatectomy.

Methods : Thirty-one patients who were not diagnosed with diabetes before pancreatectomy agreed to participate in the study. We used the FreeStyle Libre Flash Glucose Monitoring System (Abbott Diabetes Care) for continuous interstitial glucose monitoring. Capillary reference glucose levels were checked four times per day. Accuracy was checked using the Clarke Error Grid.

Results : The mean age of the participants was 56.8±12.0 years with a BMI of 23.8±3.8. 58.1% were diagnosed with malignant disease. 61.3% underwent pancreaticoduodenectomy and 38.7% underwent distal pancreatectomy. Pancreas texture, which may be related to diabetes, was soft in 38.7%, firm in 35.5%, and hard in 22.6%. The mean hospital stay was 15.5 ± 12.3 days. The clinical accuracy of continuous glucose monitoring compared to capillary glucose level was 45% in Zone A (clinically accurate zone) and 99.8% in Zone A + B (low risk of error) of the Clarke Error Gird. No device-related adverse events were reported.

Conclusions : The accuracy and safety of the Libre system were found to be adequate compared to capillary blood glucose reference values in patients who had undergone pancreatectomy. Patients felt more comfortable with the Libre system and experienced less pain. However, further studies with more data should be conducted in the future.
