[LV PP 7-S2] Advanced HCC: Results in resection of HCC larger than 10cm in a singlecenter - Western series

Background : Liver resection is considered the standard treatment for hepatocellular carcinoma (HCC), however it’s value for advanced HCC (>10cm) remains controversial. We have analyzed, in a single center, the long-term outcome associated with surgical resection in patients with such large tumors.

Methods : We retrospectively analyzed the total of 336 patient´s data who underwent liver resection for HCC between May 18th 2003 and January12th 2023. Patients were divided in two groups according to the tumor size, HCC >10cm and HCC <10cm.

Results : Median follow up was 33 months. From a total of 336 patients, 99 had a tumoral diameter larger than 10cm and 84,8% of these presented as a single nodule. All patients were submitted to a curative intent surgery and a major resection was needed in 37 patients (44%). In the subgroup of HCC >10cm, major morbidity was 7% and mortality was 6%. Overall survival was not different between the two groups (5-year survival rate 68% in large HCC vs 74%) and overall disease-free survival at five years was 43% in advanced HCC vs 52%. Advanced HCC had a recurrence rate of 47% vs 41% in HCC<10cm. From multiple independent variables analyzed, the presence of vascular invasion, cirrhosis and increased total bilirubin predicted worse overall survival (p<0.05). In a multiple variable analysis only the presence of cirrhosis was found to have an impact on survival (p<0.05).

Conclusions : Liver resection for HCC larger than 10cm is a safe and valuable option and we confirmed that it can be associated with a satisfying long-term outcome in experienced centers. It is important to do a proper selection of patients, safe techniques, and a standardized care so that adequate outcomes could be achieved.
