[LV PP 2-S2] Transarterial Chemoembolization of Hepatocellular Carcinoma before Living Donor Liver Transplantation And Post Transplantation Complication

Background : Transarterial chemoembolization (TACE) reportedly do not increase incidence of post-operative complications after deceased donor liver transplantation. However, there was no evidence in living donor liver transplantation (LDLT), in which bilio-vascular structures were small and complicated.

Methods : Retrospective cohort study was performed on 427 patients who received LDLT from 2005 to 2022 due to HCC at Severance hospital. Post-transplant bilio-vascular complications were compared between patients who received TACE (TACE group) and those who did not (no-TACE group).

Results : Total 267 patients (62.5%) received TACE before LDLT. There was no statistically significant difference of graft anatomy, such as two hepatic artery (3.0% in TACE group vs. 1.9% in no-TACE group, P=0.695), two portal vein (5.2% vs. 1.9% P= 0.142), and multiple bile duct (36.3% vs. 28.8% P=0.133). And the proportion of non hepatic artery anastomosis such as GDA, Lt gartric artery, Rt Gastroepiploic artery was similar between two groups.( 4.9% vs. 1.2%, P=0.090) Intraoperative hepatic artery re-do anastomosis rate was similar between TACE and no-TACE groups (5.6% vs. 3.8%, P=0.527). Post-operative hepatic artery complications were also similar between two groups (2.6% vs. 1.9%, P=0.870). Portal vein (2.2% vs. 1.2%, P=0.714) and bile duct complication (36.7% vs. 31.9%, P=0.364) were also similar between two groups.

Conclusions : Whether the patients with HCC received TACE or not before LDLT does not make difference in complication rate. And even if we use an appropriately selected anastomosis site, we can get comparable hepatic artery complication in multiple TACE cases.
