[LV PP 2-S1] The Effect of Supplemental Parental Nutrition in Patients after Hepatectomy

Background : Although nutritional support after hepatectomy needs caution not only for liver function but also metabolic homeostasis and immune competence, however, there is lack of established strategy and definite indication. This study aimed to assess the clinical impact of using parental nutrition concomitantly with oral feeding after liver resection.

Methods : From 2014 to 2020, 509 patients undergoing major hepatectomy in Samsung Medical Center were divided into two groups, those who start using supplemental PN concomitantly with oral feeding (n = 80) and those who did not (n = 429) and analyzed retrospectively.

Results : Of 509 patients, 80 patients use supplemental PN and 429 patients did not. There were no statistical differences with body composition between two groups. In supplemental-PN group, major complication rate was 42.5% and it was significantly higher compared to those without using PN. Subgroup analyses conducted on patients with diabetes mellitus and sarcopenia also yielded similar results. Regard of nutrition status, there was no significant difference in pre- and post-operative CONUT score changes between the two groups. Furthermore, among patients with DM and sarcopenia, there was no significant difference in CONUT score changes.

Conclusions : In terms of postoperative complications, routine use of supplemental-PN after hepatectomy is not beneficial for patients. Especially with DM and sarcopenia, its use needs caution.
