[BP BV-S4] Single Plus One Port Robotic Pancreatoduodenectomy Using the DaVinci SP System

Background : Single-port (SP) robotic systems have the advantages of multi-jointed wristed instruments and a fully wristed three-dimensional high-definition camera. Single plus one port robotic pancreatoduodenectomy(SPOPD) is rarely reported because of its complexity and technical challenges. This report aimed to anounce the feasibility of SPOPD using the new da Vinci SP system.
Methods : Patients is 65 years old women. 3cm sized intraductal papillary mucinous neoplasia with worrisome features was found in pancreas head on annual health checkup. Operation date was November 22, 2023. 2cm transumbilical incision was made and 12mm assist port was inserted at Right lower abdomen. SP slave robot was docked using floating docking technique. Assist port was used for bleeding suction and suture material, gauze, vessel look, nylon tape insertion. Stapler also used using assist port. Additional energy device such as ultrasonoic or bipolar scissors did not used.
Results : Operation time was 380minute. Perioperative blood loss was about 200ml. patient was dischared at 10 postoperative day without any complication. the pathology of tumor was revealed serous cyst adenoma. There are no specific event during 1 months after operation.
Conclusions : SPOPD is safe and feasible with regard to short-term outcomes. This technique could expand the surgical boundary of DaVinci SP. However, Futher clinical studies must be performed selective case for the advantage of using DaVinci SP.
